Thank you for letting us know something has happened. Things might feel difficult or tough at the moment but by choosing to provide your contact details we'll be able to help provide you, or someone you know, with better support.
Report & Support is here to provide advice and action when serious incidents happen to USW staff and students. If you are interested in reporting a general problem (such as issues with on campus facilities or advice on accessing particular services), please
do not
use this form and instead check
the advice zone
for the relevant contact details.
What happens next?
Once you have completed this form, if you are a student a member of the Advice Zone Team will be in touch with you. Forms submitted by colleagues within the University and PSS Ltd will be managed by HR. Completing a form through Report & Support will not start a formal complaints process. However, if you include identifying information such as someone's name, and we have concerns about risk, we may have to take action.
What happens with my data?
Your data is important to us. We won't collect or store any information that goes against our
privacy policy
